Name of Form Purpose Send To
WGSDCA Official Conformation Entry Form Use this form to enter a white GSD or a stock dog in a WGSDCA conformation show. This form can also be used to enter an obedience match held in conjunction with a conformation show. The Show Chairperson for the show (see premium or Web site)
WGSDCA Official Agility Entry Form Use this form to enter an agility fun match sponsored by the WGSDCA. The Show Chairperson for the show (see premium or Web site)
WGSDCA Official Obedience Entry Form Use this form to enter an obedience fun match sponsored by the WGSDCA. You only need this form if your dog is NOT entered in the conformation show held in conjunction with the obedience match. The Show Chairperson for the show (see premium or Web site)
WGSDCA Host Show Report Form A host show is a show held by a club other than the WGSDCA that is sanctioned by the WGSDCA. If you want your wins in the host show to be counted towards WGSDCA championship and top ten points, this form must be completed for each host show. For a complete list of clubs sanctioned to hold host shows, see the show rules.  
WGSDCA Show Reservation Form Use this form to reserve a date and location to hold a WGSDCA show.  
WGSDCA Futurity Litter Nomination Form Use this form to nominate a litter for the 2009 Futurity  
WGSDCA Membership Application Use this form to become a member of the WGSDCA  
WGSDCA Breeders Code This code serves as a pledge that a breeder will adhere to a minimum standard of ethics. Signers of the Breeders Code are indicated as such in all advertising.
